School Order Forms 2011-2012

There are three ways to use the above form:

1. Copy off this form. Fill it out completely with all information. Send in to school with payment (checks made out to “The Music Store”). We will pick up the order during our scheduled school run and deliver the items the following week.
2. Call The Music Store with your order using the order form as your guide. We will fill the order while you are on the phone with us. You can then send payment in to the school with your student or give us credit card information over the phone. We will deliver the order during our next scheduled school run.
3. Copy off this form. Fill it out completely with all information. Fax the form to The Music Store at 585-728-9981. Please be sure to include your credit card information for payment. We will fill the order and deliver it during our next scheduled school run. Please note: Which ever method you chose to use, we need the student’s name, phone number and school in case we have a question regarding your order. Orders must be received prior to 1:00 PM on Mondays in order to make Tuesday delivery and 1:00 PM on Wednesdays in order to make Thursdays delivery. School runs are as follows:Tuesdays:

   KeshequaSheet Music - The Music Store carries a wide range of sheet music and lesson books in stock. If it isn’t in stock, it can be ordered. We deal directly with several publishers including well known names such as Hal Leonard and Alfred Publications. Therefore, we can get most items within a week when not in stock.
   We also have numerous catalogs containing thousands of listings for you to browse through at your convenience to help with your selections.
